Dr. Fletcher finished his child fellowship not long after it started becoming more recognized that ADHD often persisted into adulthood. At that point most psychiatrists who treated adults did not have extensive training in evaluating or managing ADHD.

When he started his practice Dr. Fletcher was one of a handful of psychiatrists in San Francisco who had trained in adolescent and child psychiatry (where we were trained to manage clients with ADHD…including those without hyperactivity) and accepted both insurance and adult patients.

Many of his early patients were some of the software engineers fueling the tech boom. Those patients tended to be far more observant than kids and frequently took extensive notes on their experiences. The data from their observations helped him to develop a management style that keyed in on the timing of tolerance develop rather than just running the dose up until someone had side effects to achieve optimum dosing.
